HOW IT WORKS: Tell us who the appointment is for by filling out the required information below. Once you have completed that portion, select New Patient and click  Next. On the next screen, select the type of appointment type are scheduling, Adult or Child then click Next. From here, you will select your preferred day and time that works best with your schedule. Once your scheduling is complete, you will receive a notice that you need to confirm your email address. Once that has been confirmed, you will receive a follow up text message and an email to verify your appointment. We look forward to seeing you at your appointment.
